HILLINGDON is the first borough in London to introduce new technology aimed at improving the discharge of patients from hospital. 

Working with Hillingdon Hospitals NHS Foundation Trust, the council’s adult social care team and NHS staff can now access real-time information about each discharge.  

The Optimised Patient Tracking and Intelligent Choices Application (OPTICA) monitors all admitted patients and the tasks required for their discharge.

This should ensure patients leave hospital without delay, into the right care, with the right level of support, at the right time.    

The new system means staff can focus more on direct care and less on admin tasks.  

Cllr Jane Palmer, Hillingdon’s Cabinet Member for Health and Social Care, said: “This is part of our work with the NHS and other organisations to develop innovative joined-up services to meet the health, care and support needs of our residents.”
